Grant Applications, Progress Report Forms, and Final Report Forms

Welcome to the forms and grantee reporting section of our website.  Progress Reports for FY14 (awarded in March 2013) Grantees are due on September 30th, 2013.  Final reports are due on May 15, 2014. 

Small Grant (formerly "Mini-grants") report forms are due 30 days after completion of the project. Remember that we are always looking for great stories, quotes and photos to be included with your reports.

If you are a Community grantee, you need to submit a change of request program form if the Komen funded staff person changes or there is a major change to the program goals and objectives.  A budget change request form should be submitted if you anticipate changing a single line item by $1,000 or more.  All requests for grant program or budget changes are reviewed at the monthly Grant Committee meetings, which are the first Wednesday of each month.  You will be notified concerning the decision by letter shortly after the meeting.

GRANT REPORTING FORMS:
In addition to submitting an electronic copy of your progress reports, please also mail 2 hard copies to the Affiliate office: 1500 SW 1st Ave., #270, Portland, OR 97201

FINAL GRANT REPORT DUE  MAY 15, 2013:
Please send your final report in a Word document  (Do NOT make it a PDF) so that we can cut and paste your answers into the online system. Please also mail 2 hard copies to the Affiliate office: 1500 SW 1st Ave., #270, Portland, OR 97201

Small Grant Application - Due April 15 - Applications due January 15, April 15, July 15, and October 15 each year. 

FY14 Community Grant RFA - Application Deadline Passed
