FormsWelcome to the forms and grantee reporting section of our website. Quarterly reports are due for Community Grants and Transportation grants on July 31, 2009, October 31, 2009, and January 31, 2010. Final reports are due on May 15, 2010 and those forms will be posted later. Mini-grant report forms are due 30 days after completion of the project. Remember that we are always looking for great stories, quotes and photos to be included with your reports.If you are a Community or Transportation grantee, you need to submit a change of program form if the Komen funded staff person changes or there is a major change to the program goals and objectives. A budget form should be submitted if you anticipate changing a single line item by $500 or more. All requests for grant program or budget changes are reviewed at the monthly Grant Committee meetings, which are the first Wednesday of each month.
