Fundraising Form
Race fundraising deadline is October 21, 2008.
To be credited online for donations made in the form of cash or checks, please print and fill out the above“mail-in fundraising form,” attach cash or checks and mail everything to:

Approximately two weeks from receiving your form and donations, your online fundraising page will be credited with your mail in donations NOTE: the fundraising form must state clearly 1) who the donation(s) are to be credited to and 2) who the donor is.
When offline donations are entered into our online fundraising system, you will receive an email notifying you that donation(s) have been received. Your donor will also receive a receipt, if you gave us a valid mailing address for the donor. Komen Portland Race for the Cure fundraising is available until October 21, 2008. Any gifts received after that time will be processed but not credited towards your fundraising total. If you have questions regarding fundraising email us at: racecure@komenoregon.org.
